Resource Completion Form

Depending on which resource you have completed, your team or group of people you completed it with might be called something different. In some cases, your entire session will have completed the resource, in other cases it might be a smaller team or group of people or you might have completed a resource as an individual without a team participating with you . Please enter the information for EACH PERSON (including yourself) who have been participants in whichever resource you have completed. This number could range from 1 person up to 25 people.
